Hi Dr B

Can you tell us more about the genetic research you are doing? Are you using Affy or Illumina to do the genotyping? What SNPs are you working on? Thanks

The study just looked at high strength 0.1% tretinoin. It did not look at retinol so I feel it is still safe.

No risk with glycolic peels and smoking. We really do not know why they saw increased death in this study. It may be an errant finding or there may be something to it. Its not the exfoliation effect that glycolic has. Its more likely due to the effects that retinoids have on hormone receptors.

Thank you so much for your article. I was wondering if there is any concern with using glycolic peels and smoking? or using glycolic creams/alpha hydroxy creams and smoking?

If you go to skinguru.com you will see that I added a follow up blog apologizing. I have no control over what yahoo posts or reposts on the home page. Once they are written and submitted I lose control.
I personally DO NOT agree that it is beneficial to break out after a facial. The "purging" idea is not scientifically accurate. Facials cause breakouts because incorrect technique is used. Please explain scientifically for me why you believe people break out after a facial. It is not for any beneficial reason- it is either incorrect products or friction. The one exception would be if you used a retinoid which can cause breakouts but these are not routinely used in facials. You may want to go back and review your science of acne. I have a chapter on it in my upcoming book “Cosmetic Dermatology” that is the second edition and will publish in March. (You can find it at Amazon) It will be in 8 languages this time (The last one was only 4)
Do any aestheticians out there believe that it is beneficial to break out after a facial? Why do you think this? Tell me scientifically what your thoughts are.

Dr. Kirsner- so you are saying that the risk is higher even if you do not smoke and that you think that this was a chance occurence in the trial and you are not concerned. That is what several derms are saying as well. My thoughts:The product has been on the market for decades and patterns of complications have not been noticed before so this may be an errant finding. I felt that people need to know about this anyway. Thanks for your input. I know that you were one of the investigators on the trial.
